What are the steps to starting a durable medical equipment business?

I am wanting to start a durable medical equipment (DME) supply and rental business and become a provider through Medicare and Medicaid. I plan to set up a LLC. What steps should I take to get this type of business started? Also about what will it cost to start a DME business?
